Advancement to Candidacy Oral Examination

>Arrange the date and time with your advisor and the advancement committee.

>Book the room with the Undergraduate Affairs Specialist in the Math Department Office.

>Notify the Graduate Affairs Office at least 2 weeks prior to the exam date in order to post the announcement on the Mathematics Department website. Be prepared to submit the title, date, time, and room number.

Complete all 3 pages of the form including the Conflict of Interest form on page 3.

Take the completed form to the Qualifying Oral Examination and obtain the signatures of the all Committee Members. (* Asterisk to indicate Final Examination Committee Members) If the decision is favorable, obtain the signature of the Doctoral Chair of your committee, the Department Graduate Vice Chair (Graduate Affairs Officer can obtain signature) and your signature on Page 3. Take to Math Department office. Donna McConnell will take a copy for your academic file and confirm signatures have been obtained. The Advancement to Candidacy Fee of $65 must be paid at the Cashier’s Office. The cashier will stamp this form to indicate receipt of payment. The completed PhD form I, cashier’s stamp indicating payment and a completed Conflict of Interest form, constitutes an application for Advancement to Candidacy. The student’s official date of advancement is the day the completed Ph.D. Form I is received in OGS.


Final Examination For Degree of Doctor of Philosophy

>Arrange the date and time with your advisor and the advancement committee. If any of the members have changed since the Advancement to Candidacy Oral Examination, notify the Graduate Affairs Officer immediately. An exception will have to be submitted to Graduate Division and approved prior to the PhD Defense.

>Book the room with the Undergraduate Affairs Specialist in the Math Department Office.

>Notify the Graduate Affairs Officer at least 2 weeks prior to the exam date in order to post the announcement on the Mathematics Department website. Be prepared to submit the title, date, time, and room number.


Library Archives approved text and template (credit to Raphael De Santiago, former PhD student) sample bibliography file and .pdf file along with the template.

Committee members must sign the cover page.

Ph.D. Dissertation Submission Checklist

Two unbound copies of your manuscript on 8.5" x 11", 20- to 24-pound, white, bond finish, 100% cotton paper (original and a copy, without perforations or staples).

The original will have the original signatures of your committee members in permanent black ink, while the copy may have a photocopy of the signatures, as long as it is on 100% cotton paper.


Report on Final Examination for the Degree of Doctor of Philosophy Ph.D. Form II

Graduate Student Diploma and Commencement Form

The Registrar's Office uses this form to order your diploma and to place your name on the Commencement list. Spring graduates submit this form by the last working day before spring quarter begins (see Schedule of Classes). If you miss this deadline, submit it to the

Dissertation/Thesis Approval Form

Submit two copies (one may be a photocopy). Include a permanent address that will be valid for at least four months. University Microfilms International (UMI) Doctoral Dissertation Agreement Form


This form is found in the Publishing Your Dissertation brochure from UMI, which you will receive from the Library Archivist when you submit your Dissertation. Complete the entire front portion, and sign and date the back.

This agreement grants UMI the following rights:

1.The exclusive right to reproduce and distribute your dissertation in and from microform.

2. The non-exclusive right to reproduce and distribute your dissertation in and from an electronic format.

3. The non-exclusive right to reproduce and distribute your abstract in any format, in whole or in part.

These rights do not prevent you from granting other publishing rights as you choose.

ProQuest Information and Learning Company. UMI does not accept personal checks. Include your full name on the face of the money order/cashier's check.

You may purchase copies of your dissertation from University Microfilms International (UMI) at a discounted pre-publication price. The order form and list of current prices are included inside the UMI brochure. Include the cost on the same money order or cashier's check mentioned above. Degree Certification Request Form (optional)

Because it may take four to six months to receive a diploma, you may submit this form and receive a Letter of Certification that certifies your graduation. Allow a minimum of five working days for your Degree Certification request to be processed. Click on the link to
