Visitor/Appointment Request Portal

To streamline the onboarding process for your visitors or appointees, we’ve created a Visitor/Appointment Request Portal. This form collects all essential details in advance to ensure a smooth and timely arrival.

How It Works:

• You (the host) are responsible for sharing this form with your guest or appointee.
• The visitor must complete the form to initiate the appointment process.

Timeline:

• Submit at least 90 days in advance for all standard appointments.
• Submit at least 120 days in advance for visa-dependent appointments (required by UCI’s International Center).

Submitted information will be used for appointment dossiers, visa processing, and general visitor coordination.

Important:
Please share the form link with your visitor as soon as possible. Timely completion is essential for proper registration and preparation!
